Regional Chronic Disease Collaborative
Mountain-Pacific and partners have established a Regional Chronic Disease Collaborative to create a cooperative approach, leveraging the combined resources and expertise of participating members to prevent the development and progression of chronic disease.
The regional collaborative will use a learning and action network (LAN) to deliver content to health care systems and communities throughout our region.
The Regional Chronic Disease Collaborative members include:

Regional Chronic Disease Affinity Groups
As part of the LAN events, the Regional Chronic Disease Collaborative will establish affinity groups to walk participants step-by-step through the implementation of presented topics. Please contact us if you are interested in receiving the materials from one of these recent affinity groups.

November 9, 2022 – Reestablishing Team-based Care Workflows – Creativity that Leads to Sustainability
As the new normal is upon us, it is time to reengage patients in their health journey. The Regional Chronic Disease Collaborative is offering an education event that will review team-based care approaches that focus on management of chronic disease and preventative services to improve population health and patient outcomes. This event will share strategies to reestablish team-based care workflows and identify reimbursement opportunities to optimize patient engagement and reinforce the foundation of value-based care.

April 22, 2022 – Team-Based Care (TBC): Prioritizing the Well-Being of Your Team
This session will help you identify key concepts to help build, strengthen and promote resilience within care teams and work environments to help combat the ongoing strain of the pandemic. It will demonstrate how communication practices/norms can be used to encourage connection, improve relationships and promote positive and supportive interactions.

December 9, 2021 – Using Narrative Medicine for Care Team Resilience and Patient Engagement
Resilience among health care workers is a critical part of patient engagement. This event helps viewers learn strategies to help stay present and compassionate in a time of crisis and learn about using Narrative Medicine – a story that can be analyzed for meaning – as a groundwork for patient engagement. Presenters and participants shared ways to help recognize your own needs and increase the skills needed to set an environment for sharing stories that support care team resilience and that are foundational to authentic patient engagement.
June 24, 2021 – Working Together to Improve Health Outcomes
This webinar is presented by several members of the Regional Chronic Disease Collaborative. The recording focuses on ways to effectively use community, state and national resources to support the management and treatment of chronic disease, help address social determinants of health and improve health equity.

March 4, 2021 – Self-Measured Blood Pressure Program
This presentation is targeted at clinics interested in learning about implementing or enhancing a self-measured blood pressure (SMBP) program. The presentation includes the basics of implementing an SMBP program, reviewing pre-implementation considerations and an overview of available SMBP resources.
