Diabetes
More than 29 million Americans have diabetes. And 8.1 million people, or one in four, do not know it.
Working within local communities, Mountain Pacific improves diabetes health literacy, care and outcomes. By collaborating with local and national diabetes programs, we work to improve diabetes self-management and patient advocacy through education and support for Medicare patients and their families.
Chronic Kidney Disease
According to the Centers for Disease Control and Prevention (CDC), more than one in seven U.S. adults, or nearly 37 million people, have chronic kidney disease (CKD). Kidney diseases are the ninth leading cause of death in the U.S., and every 24 hours, about 340 people begin dialysis treatment for kidney failure.
Mountain Pacific is partnering with health care communities to increase screenings for early detection and improve management among patients with CKD and end stage renal disease (ESRD). We partner with providers to identify patients at a high-risk for developing kidney disease and work to improve patient outcomes.
Cardiac & Vascular Health
Heart disease is the leading cause of death for both men and women in the U.S., causing about one in every four deaths.
Million Hearts® 2022 is a national initiative launched by the Department of Health and Human Services to prevent one million heart attacks and strokes by 2022. The initiative brings together communities, health systems, nonprofit organizations, federal agencies and private-sector partners across the country to fight heart disease and stroke.
Mountain Pacific brings national initiatives like Million Hearts 2022 to the local level, working with physician offices, home health agencies, other health care professionals and stakeholders, Medicare beneficiaries and their families to improve cardiac health and quality of care.
